You'll need to visit a local independent agent who can help you research this. There is no way anyone here can properly advise you. Not all companies are in all states, and your state laws will greatly influence what the insurance companies will do in your case.
In my state ulcerative colitis will be a decline with many companies. However, depending upon the severity and if you are a surgical candidate you may be able to get a policy with conditions. These conditions can be either a rider, which means they will accept you but won't cover the colitis or anything related to the colitis, or they will fully accept you but will charge a higher premium.
Vision will be a separate policy, and in my opinion is rarely worth the money.
